Orientation Forms for All Students

Please print and/or complete the following forms and bring the entire packet to Evening Orientation on TBA. (If you are joining us after orientation, please return all forms to your Class President.) If you need copies mailed to you or have any questions, feel free to contact one of our Registrars.

1. Orientation Checklist - It may be easier to print this out to track your progress.
2. Tuition Policy - Please print and complete two copies of this form, one copy for parent to keep, one copy for KM Co-op.
3. Tuition Payment – For September and May. Please make your check out to KM Co-op Preschool.
4. Parent Participation Requirements – One copy for you and one copy for KM Co-op.
5. Emergency Information Form – With photo of your child attached.
6. Consent for Emergency Medical/Surgical Care Form
7. ImmunizationAll students must complete one of the following immunization forms:
   • a. Certificate of Immunization Status
   • - or -
   • b. Certificate of Immunization Exemption Form
8. Risk Management
   • Read the Risk Management Manual
   • Sign the Summary of Risk Managment Manual Acknowledgement Form
9. Permission to Photograph/Video
